countersigned by an official capable of legally binding the institution (e.g. Personal Investigator, Director

Order must be placed using the Reagent Request From provided here as a PDF file. Orders for reagents may be submitted with your registration documents, but they will not be processed
until your registration has been approved. All hard copy requests for reagents must be signed by the registered investigator.
